Irving R. Restituyo, MD, CMD

Chief Medical Director
Dr. Irving R. Restituyo, MD, CMD, is the Chief Medical Director of A C O Health Solutions, bringing decades of distinguished leadership in post-acute and long-term care medicine. Board-certified and widely respected in the field, Dr. Restituyo's expertise spans geriatric care, chronic disease management, and facility medical direction. His compassionate, results-driven approach ensures the highest standard of clinical excellence for every patient and facility under ACO's care. Dr. Restituyo's distinguished academic journey led him to SUNY Downstate Medical University Center, where his profound fascination with geriatric medicine took shape. Drawn to the finest institutions, he pursued advanced fellowship training in Geriatrics at the prestigious St. Luke's–Roosevelt Hospital at Columbia University; an experience that would define his life's work. Returning to southeastern Massachusetts with a clear vision, Dr. Restituyo built what has become the region's preeminent Internal Medicine and Geriatrics practice, a testament to his unwavering commitment to transforming the landscape of elderly care.

Jo Colon, MA

Jo’s journey in healthcare began in 2006 at the University of Puerto Rico in Carolina, where she first studied Business Administration. In 2008, she moved to Massachusetts to pursue the American dream and continued her education at Bristol Community College, becoming President of the International Club while pursuing Administrative Studies. During this time, Jo discovered her passion for medicine and healthcare administration — a path that would shape her entire career. She graduated as a Medical Assistant in 2011 from The Salter School in New Bedford and began her clinical journey at the Greater New Bedford Community Health Center. In 2012, Jo joined ACO Health Solutions. She became the first dual‑role Medical Assistant and Medical Scribe at the community center while completing formal medical scribe training. Her dedication, accuracy, and leadership quickly set her apart. In 2016, Jo became the Practice Manager for ACO Health Solutions’ primary care office, and by 2018, she advanced to Practice Manager of the Nursing Home Division, overseeing operations, providers, scribes, and facility partnerships across multiple SNFs. Today, Jo continues to grow academically and professionally. In 2024, she began her B.S. in Business Administration with a minor in Healthcare Management at Walden University, with graduation expected in 2027. Her long‑term goal is to pursue a license in Nursing Administration, further strengthening her leadership in healthcare operations. Jo’s journey reflects resilience, purpose, and a deep commitment to improving healthcare delivery. Her leadership keeps ACO Health Solutions organized, efficient, and aligned with the needs of the facilities and residents we proudly serve.

Helen Fabre, MHA

Assistant Manager Lead Medical Scribe
Helen Fabre serves as the Assistant Manager at ACO Health Solutions, providing essential operational support across the practice. She works closely with Practice Manager Jo Colon to ensure smooth day‑to‑day coordination, efficient communication, and consistent workflow execution.
Helen plays a key role in supervising and supporting our team of Virtual Medical Scribes, helping maintain high standards of accuracy, timeliness, and compliance in all clinical documentation. Her strong organizational skills, reliability, and commitment to excellence make her an integral part of our administrative and clinical operations.
With a calm, solutions‑focused approach, Helen helps keep the practice running efficiently and ensures that providers, scribes, and facility partners receive the support they need.

Dr. Sarah Jancek, DO

Physician, Internal Medicine & Geriatrics
Dr. Sarah T. Jancek, DO brings over 15 years of distinguished medical service to ACO Health Solutions. A 2007 graduate of Touro College of Osteopathic Medicine, her holistic, whole-body approach to geriatric and post-acute care sets her apart. Dr. Jancek excels in admission assessments, periodic evaluations, and care conference participation — ensuring seamless, comprehensive, and continuous clinical oversight for every patient in her care.

Dr. Fabian Delgado, MD, FACP

With over two decades of distinguished service in medicine, Dr. Fabian Delgado, MD, FACP, CMD brings unmatched clinical depth to ACO Health Solutions. A graduate of the Catholic University of Santiago De Guayaquil Faculty of Medicine (2002), he is affiliated with Newton-Wellesley Hospital, Sturdy Memorial Hospital, Morton Hospital, St. Anne's Hospital, and Brown Health. His mastery of post-acute and long-term care medicine — combined with his leadership as Medical Director of our prestigious CHF Program — makes him an irreplaceable pillar of excellence at every facility he serves.

Dr. Allen C. Waltman MD

Internist, Internal Medicine
Dr. Allen Waltman, MD is a distinguished Internist based in Milton, MA, bringing over 53 years of experience to ACO Health Solutions. A graduate of Boston University, Dr. Waltman is board certified in Internal Medicine and is affiliated with Brigham and Women's Faulkner Hospital, Newton-Wellesley Hospital, Beth Israel Deaconess Hospital Plymouth, and Beth Israel Deaconess Hospital-Milton. Dr. Waltman currently welcomes new patients and is committed to providing exceptional, personalized care to each individual he serves.

Adult-Gerontology Primary Care NP, AGPCNP-BC
Cindy Lang, NP is a board-certified Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP-BC) credentialed through the ANCC. A distinction that reflects both her commitment to excellence and her specialized mastery in geriatric care. Having earned her BSN from MCPHS (2009) and her MSN from the University of Massachusetts Boston (2017), Cindy brings advanced clinical leadership and dependable after-hours on-call coverage to our facilities, ensuring residents receive expert, attentive care around the clock.
Taylor Pacheco, NP is a board-certified Nurse Practitioner (AANP) who brings a sophisticated clinical focus in family and cardiac medicine. Holding a BSN from Curry College (2015) and an MSN from Simmons University (2020), she specializes in the complex management of cardiac conditions affecting the aging population. A proud member of the Sigma Theta Tau Honor Society and the American Academy of Nurse Practitioners, her academic distinction is matched only by her compassionate bedside presence.Board-certified Nurse Practitioner (AANP) with a focus on family medicine. BSN from Curry College (2015), MSN from Simmons University (2020). Specializes in complex cardiac conditions. Member of Sigma Theta Tau Honor Society and the American Academy of Nurse Practitioners.
Stephanie Clifford, NP Family Nurse Practitioner with nearly 15 years of nursing and healthcare experience. Specializes in compassionate, holistic evaluations of adult and geriatric patients, including those with complex medical and psychosocial needs. Current sub-acute and long-term care provider.
Laura Cline, DNP, FNP-BC, CPNP, RN
Laura Cline, DNP, FNP-BC, CPNP, RN attended MGH IHP from 2011-2014, graduating with a Bachelor's and Master's degree in Nursing with a focus as a Pediatric Nurse Practitioner (Sigma Theta Tau International). At Simmons University her DNP project focus was antibiotic stewardship in the outpatient setting. Her work history includes the following: behavioral health residential counselor, in-home Cognitive Behavioral therapy for children who were sexually assaulted, pediatric home care, Franciscan Children’s Hospital, Pediatric Primary Care, Pediatric Urgent Care, at Boston Children’s Hospital in the Center for Ambulatory Treatment and Clinical Research (Infusion) and Pediatric Allergy, several urgent cares throughout the greater Boston area and in aesthetic medicine (injecting Botox and fillers). In her spare time, she enjoys attending concerts/sporting events, playing in many recreational sports leagues in the Boston area (in addition to being a huge Boston sports fan), and spending time with her nieces/nephews and my family. She volunteers at the local PBS station GBH and with The Boys and Girls Club of America.
